Phone Call Error: Silence on Answer - Need Assistance

I am getting the following error when attempting to make a phone call. It will not pick up or say anything. It just answers in silence. Can someone assist or is there an issue going on? Sameer Z.

It has worked this morning, but a majority of the phone calls we make are failing in the way the task above is.
